Australia
Documents are provided by Federal (ACT), State or Territory Police Forces.
National Police Clearance Certificate can be issued by the Federal Police (AFP) in Canberra that lists criminal records in all states and territories, with the eception of Queensland.
State or Territory Police Force (see address list below): NOTE: State Police Clearance Certificates can only be issued to the applicant and only cover criminal records in that state. Some states now provide National POlice Clearance Certificates too.

United Kingdom
Form is available at http://www.met.police.uk/dataprotection/ Apply to your local police station if living in the UK. The National Identification Service (NIS) based at New Scotland Yard, London, carries out these checks on behalf of all Constabularies in England and Wales. If you live or have lived at some time in the previous 10 years, in Scotland you will have to apply separately to the appropriate Scottish police force. Alternatively, the application can be completed by another person, perhaps in the UK, with the written authority of the subject. This is what to do:
* Go to ‘UK Police Forces.’ at http://www.police.uk/forces/default.asp
* Then, go to ‘List all UK Forces.’ The relevant Force’s website can be accessed from this screen.
* Once into the Force’s website, go to ‘subject access,’ or ‘Freedom of Information.’
* The subject access page will explain the procedure, allow an application form WA162 to be downloaded, and explain how to complete and submit the form, photocopies of two documents that prove your identity and fee of £10.

These a re the addresses fo gettiing a State Police Clearance Certificates for the U.S.A. Please note that Thai consulates may also require an FBI Clearance from the Federal Bureau of Investigation. Not all Thai consultes operate the same requirements. The following are required in order to obtain a copy of your FBI records:
* Your fingerprint card.
* A note requesting a copy of your FBI record.
* A certified check or money order in the amount of US$18 made payable to the Treasury of the United States. If made payable to the FBI the fingerprints will be rejected.
